1. Introduction
1.1 This policy relates to the recording and reporting of injuries sustained during Stokesley Hockey Club activities, including:

Other organised hockey activity
1.2 For the purposes of this policy, an injury is defined as any physical injury sustained during hockey activity that may:

Stop or restrict participation

Require medical attention

Prevent future hockey activity

Affect normal daily activities
1.3 Stokesley Hockey Club is committed to ensuring the safety and welfare of all participants involved in hockey activities.
1.4 The club recognises the importance of injury reporting in:

Supporting safer hockey participation

Managing return-to-play processes

Supporting safeguarding and welfare responsibilities

Meeting health and safety obligations
1.5 Stokesley Hockey Club uses its own Injury Reporting Form and reporting procedures to record injuries and incidents occurring during hockey activity.

2. Purpose of Injury Reporting
2.1 The purpose of injury reporting is to:

Ensure injuries are managed appropriately

Record relevant accident and injury information

Promote player welfare and safety

Support appropriate medical follow-up

Improve understanding of injury trends within the club

Assist in preventing future injuries
2.2 Injury reporting also assists the club in:

Meeting safeguarding responsibilities

Supporting insurance and legal requirements

Maintaining accurate health and safety records

3. Injuries That Must Be Reported
3.1 An Injury Reporting Form must be completed for any player or participant:

Removed from play or training due to injury
-
Reporting an injury immediately after hockey activity
-
Reporting an injury sustained during hockey that affects later activity or participation
-
Requiring treatment from a first aider, medical practitioner or hospital
-
Suspected of having suffered a concussion or head injury
-
Sustaining any injury requiring emergency medical assistance
3.2 Injury reporting applies to:

Senior players
-
Junior players
-
Coaches
-
Officials
-
Volunteers
-
Spectators injured during club activity where appropriate

4. Injury Reporting Procedure
4.1 In the event of an injury during hockey activity, the following procedure should be followed:
4.1.1 Ensure the injured person is safe and remove them from activity if necessary.
4.1.2 Seek appropriate medical attention where required:

First aid

GP
-
Accident & Emergency
-
Emergency services (999)
4.1.3 Inform the relevant team captain, coach or club official.
4.1.4 Complete the Stokesley Hockey Club Injury Reporting Form as soon as reasonably practicable.
4.1.5 Where the injured person is under 18 years of age:

Parents or guardians must be informed as soon as possible.
5. Responsibilities
5.1 Club Committee
The Club Committee is responsible for:

Supporting safe hockey participation
-
Ensuring injury reporting procedures are followed

Reviewing injury trends where appropriate
-
Maintaining appropriate reporting records
5.2 Coaches and Captains
Coaches and captains are responsible for:

Responding appropriately to injuries
-
Ensuring injured players are removed from activity where necessary

Supporting the completion of injury reports

Escalating serious concerns appropriately
5.3 Players and Members
Players and members are responsible for:

Reporting injuries honestly and promptly

Following medical advice

Not returning to play when unsafe to do so

6. Concussion and Head Injuries
6.1 All suspected concussions or head injuries must be treated seriously.
6.2 Any player suspected of concussion:

Must be removed from play immediately

Must not return to hockey activity the same day
-
Should follow the club’s Concussion Policy and appropriate medical advice
6.3 A concussion or head injury must always be recorded using the Injury Reporting Form.
7. Young People (Under 18)
7.1 The club has a duty of care towards all young people participating in hockey.
7.2 Parents or guardians must be informed of:

Any significant injury

Any head injury or suspected concussion

Any injury requiring medical treatment
-
Any situation involving emergency services
7.3 Relevant medical information and emergency contacts should be provided to the club by parents or guardians wherever required.

8. Record Keeping and Confidentiality
8.1 All injury reports will be:

Handled in accordance with data protection legislation
8.2 Information will only be shared on a need-to-know basis for:

9. Return to Play
9.1 Players returning from injury should:

9.2 The club reserves the right to restrict participation where player welfare or safety may be at risk.

10.1 Failure to comply with this policy may:

10.2 Where appropriate, failure to follow injury reporting procedures may result in:
